Convalescent Care Center

The Convalescent Care Center serves 110 residents over the age of 65. Most Geriatric residents needs can be met at the C.C.C. with the exception of ventilator dependent or residents with a psychiatric diagnosis. When a resident's level of care exceeds our capabilities, the resident is transferred to a higher level of care. Nursing care is provided 24 hours a day, 7 days a week, 365 days a year. The C.C.C. has fully staffed departments in Nursing, Dietary, Social Services, Activities, Environmental Services, and Administration. Other services such as Physical Therapy, Occupational Therapy, and Speech Therapy are contracted with outside agencies. All residents are assessed using the Initial assessment, weekly summary, and the Minimum Data Set. Assessments are completed on admission, some are daily, all are weekly, quarterly, annually, and on change of condition.

ADMISSION INFORMATION

AVAILABLE SERVICES
Lompoc Valley Medical Center Convalescent Care Center (CCC) provides services to convalescing and/or chronically ill individuals 65 years of age and older. Basic services include restorative care, nutritional services, therapies, administration of medications, and activities of daily living.
If applicant needs additional services other than those described, or would like admission prior to the age of 65, please make an appointment to speak with Judilee Smith, R.N., CCC Administrator, before applying.
ADMISSION DETERMINATION
Admission to the CCC is determined by:
- CCC's ability to provide needed care to the prospective resident.
- Validation that the prospective resident has a primary care physician with admitting privileges at Lompoc District Hospital.
- Approval for reimbursement from the prospective resident's payer.
- Date of receipt of the valid application.
Please Note: Payment source is not a factor in determining admission.
PAYMENT FOR SERVICES
Daily Charge: Two hundred-fifty ($250.00) per day for a two-occupancy room. Prescription medications, physician services, and therapy services are not included in the daily charge.
Payment Source: The District accepts Medicare, Medi-Cal, all major health plans or private funds. Eligibility must be verified' with the payment source and payment for services finalized before admission.
If your payment source is:
Medicare
Short stay SNF Medicare Services: Medicare covers 100 days per benefit period, provided there is a need for daily skilled care and the resident does not refuse the skilled care that is ordered by the physician. Medicare covered services include: semi-private room, meals, routine nursing care, rehabilitation therapies and services, drugs furnished by the facility, medical supplies and use of equipment. The first 20 days are paid at 100% by Medicare. After the 20th day the beneficiary is billed a co-pay of $119.00 per day. If your goal is to return home we will assist you to achieve this goal as quickly as possible.
Medi-Cal
Applicant must present their valid Medi-Cal identification card before admission.
Private Pay
Applicant must pay in advance for two months and provide information at the time of registration to verify their ability to continue to pay for services provided.
Insurance Plan
Applicant must pay in advance any difference between our daily rate and what applicant's policy allows.
In the event the applicant is not competent to conduct their own business affairs, satisfactory alternative arrangements must be made.
Applicant's financial matters should be in order before admission as it is the District policy not to admit prospective residents if their payment source has not been finalized.

ADMISSION PROCESS

The application form will not be accepted until the applicant's primary care Physician, with admitting privileges at Lompoc District Hospital has been verified.
Categories of Admission
There are two categories of individuals desiring admission
Active Status: Applicant desires immediate admission. CCC will notify applicant or representative when accommodations are available.
In-Active Status: Applicant desires future admission. Applicant or representative will notify CCC when they desire to be on active status.
Waiting List: Applicant's name will be placed on the waiting list based on the date the application was received and whether or not they are active status or inactive status. The date received always determines the place on the waiting list.
Admission Date It is difficult to accurately predict an admission date as an applicant may move from inactive status to active status. Therefore, counting those ahead of the applicant on the waiting list is an effective tool for estimating an admission date.
Admission Inquiry and Response
When an active status applicant's name reaches the top of the waiting list, the CCC will notify applicant or representative when accommodations are available and inquire about applicant's desire for immediate admission. Response to this inquiry within 24 hours after contact is important or the next applicant on the waiting list will be contacted.
Applicant may accept admission when contacted and the admission process will begin with an assessment to ensure that the CCC can meet the applicant's needs. Admission is then likely to occur within the next two to three days. Applicant may decline admission when contacted if the~1 do not desire admission at that time and may be asked to place the application on inactive status. The applicant or representative would be responsible to notify the CCC when they desire to on active status. The original application date received will remain the same even if applicant declines admission or if applicant is admitted for a time and later discharged.
Having an application on file does not guarantee admission.
Application Updates
Applicant or representative may contact the CCC to update the application information at anytime. However, every two years the applicant or representative will be notified via mail to confirm their wish for us to retain the application. They should review and update information and return the signed cover letter and accompanying application within 30 days.
